Pityriasis versicolor

a recurrent disease that affects only the horny surface of the epidermis. The pathology is called “lichen versicolor” because the spots have colors: yellow, whitish, brown. The skin begins to peel off and spots appear in the summer in children and adults. Residents of countries with high humidity and warm climates are susceptible to pathology.

In Russia, skin formations appear in 15% of the population. These are mainly people from 15 to 40 years old, more often observed in men. Children under 7 years of age rarely get shingles. For treatment, contact a dermatologist at our clinic, he will conduct an examination and prescribe medications that will improve the patient’s condition.

Types of pityriasis versicolor:

  • Spotty-flaky. During the period of spread, it affects the body in different places: behind the ears, in the groin areas.
  • Erythematosquamous. Often observed in adults and children. The rash is beige or coffee-colored, without inflammation. At first the formations are small, but soon they increase in size. This type appears on the head. It worsens in autumn and spring.
  • Follicular. Lesions appear on the legs, back, chest, and arms. The spots spread in the hair follicles, their size is up to 3 mm, and itching is observed. This form can form during antibacterial therapy or the use of steroids in people with diabetes.
  • Inverted. Characterized by peeling, itching, redness. This species is not common. When the disease occurs, symptoms are not expressed; spots appear on the head.

If you experience formations on the skin, it is better to seek help from qualified specialists at our clinic. Quickly identifying the disease in its early stages will help prevent its spread.

Herpes zoster

It is caused by a virus that remains “dormant” in the human body for the time being. It appears in the form of fluid-filled blisters, usually located in the intercostal part of the body. Associated symptoms of herpes zoster: pain (caused by damage to the intercostal nerve endings) and itching of the skin in the affected area; sometimes fever with chills, headaches.

Treatment of herpes zoster is complex. Since the blisters of shingles burst, they are initially treated with brilliant green. And to lubricate the crust that forms in their place, use dermatol ointment. In parallel, the doctor may prescribe antipyretic, painkillers and antiviral drugs, quartz and laser therapy.

Symptoms of pityriasis versicolor:

  • Formation of pink spots. At first they are single, then they increase in size and merge. Rashes are observed on the stomach, neck, under the hair, on the chest.
  • As the disease spreads, they become dark in color. May be red or brown. The colors vary on the affected areas, which is why lichen is called tinea versicolor. In some cases, the formations merge into one whole.
  • After a certain time, peeling occurs. The scales resemble bran.
  • Itching is observed, but not in all cases.
  • After healing, white or light spots remain on the skin; tanning does not affect them; this is caused by cell damage as a result of fungi.

Do stains go away on their own?

Sometimes pink marks after rashes disappear after a short time. This happens if the rash with lichen was not pronounced and rather superficial. Brown spots that arise after long-term treatment or severe illness may not disappear for years, and some remain forever.

Sometimes scars appear at the site of skin lesions. Small scars may disappear, but fairly noticeable scars require treatment.

More pronounced spots and scars on thin facial skin. Such dark marks cause discomfort and therefore require measures to be taken to remove them.

Clearwin is a cream that breaks down blemishes from the inside and helps renew skin cells.

Diagnosis of pityriasis versicolor

The disease is diagnosed by a dermatologist. He performs a visual examination of the patient, studies the presence of spots and their distribution, and makes a diagnosis at this stage. If the form of lichen is not pronounced, then the clinic specialist uses the following methods. Using a Wood's lamp, he makes several tests. After highlighting areas of the skin, they may change color to yellow-red or green.

To clarify the diagnosis, laboratory diagnostics will be required. Specialists using special equipment will be able to detect the presence of the disease after additional research.

Ringworm

The source of trichophytosis - this is the second name for ringworm - can be any of the fungi that primarily affects the hairy parts of the body (head and beard), neck, shoulders, face. The symptoms of this type of lichen are usually as follows: first, swollen pink or red spots with clearly defined edges appear, then the edges of the spots become covered with bursting blisters and an itchy crust, and finally, the center of the spot begins to peel off, and the hair in the affected area breaks off and thins.

Ringworm is diagnosed using a Wood's lamp. If the disease is confirmed, the patient - children often suffer from ringworm - is protected from contact with others and is prescribed complex therapy, including taking special tablets (for example, Terbinafine) and the use of antifungal ointments. In particularly difficult cases, it is possible to take hormone-containing agents that help stop the inflammatory process on the skin.

Differential diagnosis of pityriasis versicolor with other diseases

To make a diagnosis, the doctor performs the following procedures:

  • Conducting an examination under a microscope to determine the scraping. The specialist scrapes the scales from the skin area, processes it and examines it. Using a microscope, it is possible to detect spores and fungi that are round in shape. The mycelium threads are white in color and long in shape, like a thread.
  • Balzer test. A 5% iodine solution is used, which is applied to or near the infected areas. This study will take time; the affected skin gradually acquires a different color.

Causes of white lichen

No specific causes of pityriasis alba have been identified. This disease is not contagious. There is some association with a history of allergic diseases in patients. In such cases, pityriasis alba is classified as a type of atopic skin lesion. However, sometimes such rashes occur in people without a history of allergic conditions.

Foreign dermatologists believe that pityriasis alba is a nonspecific inflammation of the skin with residual hypopigmentation. This means that after the peeling goes away, for some reason the number of melanocytes in the skin decreases, and the area remains pale even during the tanning process. When analyzing the affected areas under a microscope, atrophy of the sebaceous glands is also revealed [2].

Some sources cite infection with Malassezia yeast as the cause of pityriasis alba. This fungus produces a specific substance that affects skin enzymes and causes a disruption in the synthesis of the melatonin pigment. As a result, some areas of the skin become pale and do not tan. However, there is no reliable evidence for the hypothesis of the fungal origin of pityriasis alba in the scientific literature [1,3].

Lichen alba often occurs against a background of tanning, especially in combination with dry skin and atopic dermatitis. It is difficult to identify cause and effect here. It is likely that the tanning of the surrounding skin simply makes the affected areas more visible.

Researchers have not found an association of pityriasis alba with UV exposure, frequent bathing, and serum copper deficiency [2,3].

Treatment of pityriasis versicolor

The dermatologist prescribes treatment depending on the nature and form of the disease, the characteristics of the lesion, the age of the patient, and the presence of other pathologies. Applies complex treatment, which includes ointments, sprays, creams, shampoos. They consist of components that effectively affect the disease. Depending on the complexity of the pathology, the patient is prescribed tablets. Therapy will help overcome lichen in a short time and eliminate complications.

For treatment, the doctor prescribes ointments that contain terbinafine. Treatment of the affected skin area is prescribed 2 times a day. This substance quickly destroys microbial cells and eliminates the appearance of new formations. Imidazole, clotrimazole and others are effective for treatment. The substances are included in creams and gels. Tolcyclate and ciclopirox are ointments prescribed to ensure that the pathogens quickly die.

When lichen appears in the hair, the doctor prescribes shampoos containing ketoconazole or nizoral. In the initial stages, patients are recommended salicylic and zinc ointments. The products quickly relieve itching. You should not use medications without consulting a specialist. Qualified doctors of our clinic, using modern equipment, will diagnose and prescribe a course of treatment.

A visit to a dermatologist is necessary 21 days after the prescribed treatment. The doctor will assess the patient’s condition, prescribe additional medications and preventive measures, these include physiotherapy and others.

Treatment options

You can get rid of stains with the help of drug therapy, traditional methods and cosmetic procedures. The sooner action is taken, the more effective the treatment will be.

Drug therapy

There are many drugs that can eliminate skin pigmentation after rashes. Among them are:

  1. "Antiscar". A product for topical use that contains components that destroy melanin. At the same time, the cream has anti-inflammatory, regenerating and moisturizing effects.
  2. "Clearwin." A cream that breaks down blemishes from the inside and helps renew skin cells.
  3. "Pentanol". A product that renews the upper layers of the skin, heals wounds well and eliminates old scars.
  4. "Mederma". The gel has renewing properties. During its use, pigment marks on the skin significantly lighten to the color of the body.

Ointments are good for restoring skin color:

  • salicylic;
  • heparin;
  • synthomycin;
  • zinc

Mederma gel

To lighten age spots, various cosmetic substances are used, which include:

  1. Azelaic acid. This component helps lighten stains and has exfoliating properties.
  2. Water-soluble alpha hydroxy acids. They remove pigmentation perfectly and have a targeted effect.
  3. Arbutin. Cleanses the skin, making it bright.

ethnoscience

To lighten the marks left after lichen, various folk recipes are used:

  • Parsley. Chop one bunch of greens and pour 250 grams of boiling water. The hot mixture should be allowed to steep for thirty minutes. The strained decoction is used to treat skin spots every day at least three times a day.
  • Herbal mixture. Pour one tablespoon of celandine, oak bark, chamomile into 500 grams of hot water and keep in a water bath for ten minutes. Strain the cooled liquid and treat pigmented areas in the morning, afternoon and evening.
  • Alcohol infusion. One glass of medical alcohol requires two tablespoons of St. John's wort. This mixture should be covered with a lid and placed in a shaded place for ten days. After this, the infusion is filtered and used for daily treatment of spots after rashes.
  • Honey with cinnamon. These products are mixed in equal quantities. Apply a mask of honey and cinnamon to the pigmented areas for fifteen minutes daily. For sensitive skin, it is better to repeat the procedure after twenty-four hours.

Prevention of pityriasis versicolor

To reduce the disease, preventive measures play an important role. They include regular washing of skin areas, removal of sweat, sebaceous fat, and regular change of bedding and clothing. In hot weather, antifungal shampoos are an excellent method. They will eliminate the occurrence of relapses. The specialist prescribes products for regular hair washing. As a preventive measure, it will take 5 to 7 days to use the drugs. The skin should be wiped with salicylic alcohol every day.

It is better to wash things not with powders, but with soap, adding soda to the water. You can avoid deprivation; for this you need to strengthen your immune system, eat right, and take vitamins once a year. Experts recommend playing sports and taking walks in the fresh air for treatment. In case of illness, it is recommended to include vegetables, herbs, fermented milk products in food, and exclude spicy, sweet and smoked foods.

Gastrointestinal disorders can cause pityriasis versicolor, so you need to undergo a course of treatment.

What can be confused with pityriasis alba?

In some cases, it is necessary to differentiate lichen alba with the following pathologies:

  • vitiligo;
  • contact and atopic dermatitis;
  • post-inflammatory hypopigmentation of any origin (including after the use of drugs);
  • fungal infections;
  • shingles;
  • depigmentation of nevus (anemic nevus);
  • psoriasis;
  • seborrhea;
  • pityriasis rosea;
  • discoid and nummular eczema [1,3].

Complication of pityriasis versicolor

The pathology affects the upper layer of the skin, but is often severe. It affects internal organs.

In case of severe itching, the patient scratches the severely damaged skin. All this causes discomfort and leads to inflammation, in addition to infectious pathology. Contact a specialist immediately. You can’t just start the disease and start solving the problem. A specialist will help and prescribe medications to improve the condition.

If the disease returns after all measures, we recommend that you undergo examination by our specialists.

Prevention

Lichen stains are quite difficult to remove. Therefore, rather than treating pigmentation for a long time, it is better to prevent its occurrence. Prevention of skin spots includes the following rules:

  • exclude self-medication of deprivation;
  • prevent the development of the disease and complications;
  • adhere to hygiene rules;
  • do not scratch the affected areas even if there is severe itching;
  • do not use cosmetics that increase the inflammatory process and cause an allergic reaction;
  • at the slightest manifestation of the disease, immediately consult a doctor for help;
  • do not consume allergenic products that intensify the process of skin rashes;
  • do not rip off dry crusts while the affected areas are healing;
  • follow all doctor's orders.

Pityriasis versicolor in children

The disease is not typical for children under 6 years of age. During this period, the skin has protection mechanisms that prevent the dermis from being exposed to moisture and high temperatures. However, if caring for the baby is not correct, parents lubricate the skin with oils: shea butter, cocoa, then this disrupts the functioning of the defense mechanisms. For this reason, the disease appears in preschool age.

In children during the prepubertal period, hormonal changes begin in the body. The skin is very sensitive to high humidity and exposure to the sun, like adults. Thanks to changes, fungi can provoke the appearance of disease.

Ringworm in children occurs with the same symptoms as in adults. The treatment is no different. If redness appears, it is advisable to immediately consult a doctor. The specialist will conduct an examination and prescribe a drug that is suitable for the child’s age. It is not recommended to use the products without the advice of a specialist, as this can lead to negative consequences.

How to remove white spots from ringworm

Many people find out that they have pityriasis versicolor only after white, discolored spots appear on the skin. In this case, there is no need to worry: it just means that the fungus has already been destroyed and a cure has occurred. The fungus that causes tinea versicolor interferes with the functioning of melanocytes, the cells responsible for skin pigmentation. Therefore, especially if you spend a long time in bright sun, previously affected areas appear discolored. Usually the white spots disappear after 3-4 tanning sessions.

Pityriasis versicolor during pregnancy

As a result of hormonal changes, the appearance of pathology is possible, but it does not harm the mother and fetus. The disease causes discomfort, but self-medication and folk remedies are unacceptable. If pathology appears, contact a dermatologist, he will choose treatment and, if necessary, prescribe diagnostics. Pregnant women are prescribed therapy for ringworm, taking into account the danger of the situation.

The doctor prescribes drugs that do not enter the bloodstream, does not prescribe anti-fungal tablets, they are toxic and harmful to the unborn child. Nizoral is prescribed; it is not recommended to use the medicine without a prescription.

Many dermatologists advise delaying the full course of treatment until after the baby is born. The fungus is not harmful to the fetus and will not affect its development. Oral medications are prohibited during breastfeeding, as they can pass into the milk and harm the baby's health.

Diagnosis of lichen alba

Patients turn to doctors on their own, as in most cases they are concerned about the appearance of unusual rashes that cause them anxiety. Usually they turn directly to dermatologists, less often to pediatricians or therapists. Making a diagnosis, as a rule, does not cause difficulties for a specialist, since the symptoms of pityriasis alba are very characteristic. Already at the first appointment, without resorting to additional examination methods, the dermatologist determines this type of lichen.

How can the diagnosis be confirmed? For this, additional types of diagnostics are used:

  • examination of the spot under a Wood's lamp to exclude vitiligo: lichen alba does not fluoresce, the color of the spot does not change, hypopigmentation does not increase;
  • Dermatoscopy for this type of lichen reveals an area with unclear boundaries and slight peeling, the hair color in the area of the spot is the same as on the rest of the body;
  • reaction with KOH solution to detect fungal infections is negative;
  • microscopy and culture for mycoses are negative;
  • A biopsy is not usually performed, but if done, a decrease in the number of melanocytes and the absence of traces of infectious agents can be detected [2,5].

Treatment of white lichen

We hasten to please patients: pityriasis alba is considered a benign disease and goes away on its own even without treatment. Another thing is that this will take time: months or even years. If the stains are on areas of the skin hidden by clothing, this does not raise any questions. But if the rash is located on the face and neck, then it is more difficult to tolerate. Especially in adolescence and young adults, when increased attention is paid to aesthetics.

How to treat pityriasis alba? General recommendations:

  • cover the rash from direct sunlight (hypopigmented spots contrast too much with tanned skin, sunburn is possible at the site of depigmentation);
  • use moisturizing creams to reduce flaking;
  • Apply sunscreen to exposed areas before going outside.

If the lesions are extensive or you don’t want to wait for the pityriasis alba to go away on its own, you can use the types of treatment recommended by your dermatologist.

Consultation with a doctor for lichen alba

If you suspect that unusual spots on the skin are symptoms of pityriasis alba, you should definitely consult a doctor - a dermatologist or therapist. Despite the fact that this lichen goes away without treatment, other diseases must be excluded.

During the appointment, the doctor will be interested in how the spots changed, after which they appeared, whether there is itching and discomfort, what the person was treated with and how effective these methods were. In addition to a targeted examination of the rash, the doctor may palpate the regional lymph nodes, ask to see the mucous membranes, and check the condition of the nails. If there is doubt about the diagnosis, then examination under a Wood's lamp, dermatoscopy, and scraping are used [1].

Mycoses of the feet

Mycosis of the feet is an extremely common disease. Infection occurs in saunas, swimming pools, gyms and other places where a piece of the patient’s skin can get on the skin of a healthy person. The disease manifests itself as peeling in the area between the toes, which later transforms into erosion. Blistering, peeling, deformation of the nail plate, and brittle nails may occur.

The main preventive measure is to use only your own shoes and under no circumstances walk around the pool or sauna without them. Dry your skin especially thoroughly after visiting baths and swimming pools. The fungus can live in a rubber mat or on a wooden grate, so it's best to avoid these places entirely.

Prevention of ringworm and other mycoses

  • Never touch other people's animals.
  • Avoid tactile communication with sick people and animals.
  • Do not allow your pets to interact with other people's animals.
  • Do not use other people's combs or towels.
  • Do not touch common furniture or curbs with your naked body.

After diagnosing mycosis of the feet and nail plates, disinfect shoes and wash socks at a higher temperature (min. 60 degrees). In summer, you should wear light, breathable shoes so as not to create favorable conditions for mycosis.

Finally, it is worth mentioning that foot hygiene is an important element of prevention. Avoid overheating your feet by wearing shoes only and drying your toes thoroughly after swimming. In addition, you must use only disinfected nippers and files.

What are mycosis and ringworm?

The content of the article

Fungi are the cause of many infections in humans and animals. Ringworm, also classified as mycoses, is a fairly common disease.

Among all mycoses, the most common is athlete's foot, which is estimated to account for 15-30% of all cases. Yeast infections (candidiasis) are also on the rise due to the widespread use of antibiotics, corticosteroids, immunosuppressants and the spread of immune-killing HIV infection.
